Conditions of Admission Document - Regulatory Source

Can someone point me in the right direction for finding the regulatory source for the requirements to provide Conditions of Admission to hospital patients? Is this a state regulation? CMS? Conditions of Participation? I can find reference to providing Conditions of Admission to patients in the Medicare Manual but those citing are around waivers, etc.

    It is essentially the consent to treat form signed upon admission. Typically contains the following types of elements:

    Consent to Medical and Surgical Procedures

    Nursing Care

    Physicians are Independent Medical Practiioners

    Personal Belongings

    Maternity Patients

    Participation in Medical Education/Teaching Programs

    Consent to Photograph

    Financial Agreement

    Assignment of Insurance Benefits

    Health Plan (Insurance) Obligation

    Acknowledgements

    Lab Test Result Acknowledgement

    Signature LInes
