Professional billing for 76492
The Podiatrist, in our Wound Clinic, uses ultrasound guidance when doing some procedures. He is not certified or credentialed in Radiology although trained in using the ultrasound guidance. Can he bill for 76492?

If the credentialing process at the facility requires this for using ultrasound guidance, then this would not be reportable. The biggest question is, based on CPT guidance, are permanent images stored in the patient record? CPT guidelines for the ultrasound section state: "Ultrasound guidance procedures also require permanently recorded images of the site to be localized, as well as a documented description of the localization process, either separately or within the report of the procedure for which the guidance is utilized."
We find that this is the missing piece in most non-radiology departments - there is a hand-held ultrasound machine that allows guidance, but does not create a permanent image to be stored.
