Billing facility cardiopulmonary rehab services provided by certified exercise physiologist

In a CAH, is it appropriate for an outpatient hospital to bill facility charges for cardiopulmonary rehab services (CPTs 93798, 94625, G0237) provided by a certified exercise physiologist (ACSM-EP) under the indirect supervision of an MD, DO and direct supervision of an RT?
